High AST in dogs can look alarming on a bloodwork report, especially when the value is highlighted or marked high. Lab reports can look clinical and impersonal, yet they carry emotional weight when they involve someone you love. You may be wondering if the result means liver disease, muscle damage, or something far more serious.
AST is a clue, not a diagnosis. It is one piece of a much larger medical picture, and a high AST alone does not automatically mean liver failure. In many cases, it reflects something temporary, mild, or even expected.
What is Aspartate Aminotransferase (AST)?
Aspartate aminotransferase, abbreviated AST, is an enzyme — a specialized protein that helps chemical reactions occur inside cells. AST plays an essential role in amino acid metabolism, moving nitrogen between molecules so the body can use amino acids efficiently for energy production and tissue repair.
Under normal conditions, AST lives inside cells — in high concentrations in the liver, but also abundant in skeletal muscle, heart muscle, and red blood cells. Because it exists in several tissues, AST is not liver-specific. When cells are damaged or stressed, AST leaks into the bloodstream.
In a healthy dog, only small amounts of AST circulate in the blood. Those low levels reflect normal cellular turnover. Cells naturally age and are replaced. Blood testing simply measures how much AST has escaped into circulation at the time the sample was taken.

What Does High AST in Dogs Actually Mean?
When AST is elevated, it tells us that somewhere in the body, cells containing AST have been disrupted. The keyword here is disruption, not destruction. A high AST indicates cell injury, not necessarily permanent damage. The elevation may be mild and temporary, or more significant and ongoing. Context matters enormously.
Because AST is found in both liver and muscle, veterinarians never interpret it in isolation. If AST is high but another liver enzyme such as ALT is normal, we begin thinking about muscle sources. If both AST and ALT are elevated together, the liver becomes more likely. If AST rises along with creatine kinase, a muscle-specific enzyme, that strongly suggests muscle injury.
It is also important to remember that AST does not tell us severity by itself. The number alone does not predict outcome. A moderate elevation can occur with something as simple as recent strenuous exercise or a minor muscle strain. At the same time, higher values may signal more significant disease. Trends over time help clarify the picture.
Most importantly, AST is not a diagnosis. It is a biochemical signal that prompts further evaluation. It guides us toward questions rather than conclusions.
Why Does AST Rise, and What Are the Symptoms
AST rises for one fundamental reason: cell membrane integrity has been compromised. When liver cells or muscle cells become inflamed, stressed, injured, or deprived of oxygen, their membranes become “leaky,” allowing AST to escape into the bloodstream.
- Organ disease — liver inflammation, infection, toxin exposure, or reduced blood flow; or muscle inflammation, trauma, or degenerative muscle disease
- Systemic illnesses — severe infection, heatstroke, or shock causing widespread cellular stress
- Medications and toxins — certain drugs like anticonvulsants or chemotherapy agents, or toxins such as xylitol or specific mushrooms
- Metabolic conditions — hypothyroidism or severe electrolyte imbalances that indirectly stress muscle or liver tissue
- Temporary stressors — strenuous exercise before bloodwork, recent muscle injections, minor trauma, or a difficult blood draw
Symptoms depend entirely on the underlying cause. If liver disease is present, you might see decreased appetite, vomiting, diarrhea, jaundice, or lethargy. If muscle injury is responsible, you may notice stiffness, weakness, reluctance to move, or pain. However, many dogs with mildly elevated AST show no visible symptoms at all. A dog can feel completely normal despite a moderate AST increase. That is why routine screening bloodwork often uncovers it incidentally.

What Can Tell Me More?
When I evaluate an elevated AST, I never stop at that single value. I look at patterns. ALT, alkaline phosphatase, bilirubin, and albumin help assess liver health. Creatine kinase helps determine whether muscle damage is involved. A complete blood count may reveal inflammation or anemia. Urinalysis provides insight into kidney function and metabolic balance.
Imaging studies, such as abdominal ultrasound, allow us to visualize liver structure. In some cases, bile acid testing or more advanced diagnostics help evaluate liver function more directly. If muscle disease is suspected, additional blood markers or even a muscle biopsy may be considered.
Equally important is trend analysis. Repeating the blood test in two to four weeks often clarifies whether the change was temporary or persistent. If AST normalizes on recheck, the cause was likely transient. If it continues to rise or remains elevated, further investigation becomes more appropriate.
Your veterinarian will also integrate your dog’s age, breed, medical history, medications, lifestyle, and physical exam findings. Laboratory values only gain meaning when interpreted within the full clinical context. That is the art and science of veterinary medicine.
